The Vineyard Nursing Association is Martha's Vineyard's only Medicare Certified Home Health agency, providing a full complement of Home Care Staff who deliver customized health care for short term, long term and chronic illness through the following programs and services.
VINEYARD NURSING ASSOCIATION MAJOR PROGRAMS
Skilled nursing services are based on a comprehensive, individualized nursing assessment and a team approach including the patient, the physician and the home care staff. Advanced technologies enable RNs to use sophisticated high tech medical equipment and procedures in the home to promote healing and provide comfort.
Specialized and advanced care programs are provided by professionals with extensive home care experience and advanced training which include: wound care management, home IV therapy, Cardiac care program including EKGs, diabetes management, maternal and child health, osteoporosis screening and management, tele-monitoring, pain management and palliative care
Certified home health aides provide assistance with basic needs for living at home such as bathing, dressing, and other personal care needs, meals and homemaking services
Rehabilitation services are provided on referral or as a continuation of services from hospital to home settings. Therapists work in concert with the other health team members, to guide the patient to meet his or her goals toward independent living. Physical therapy focuses on increasing strength, endurance, and mobility to maximize safe and functional independence within the home and community. Occupational therapy focuses on increasing independent skills for daily living such as bathing, toileting, dressing and cooking. Speech therapy focuses on the evaluation and treatment of swallowing problems and speech deficits.
Medical Social Work services are provided to help the individual and family cope with the impact of illness through access to and coordination of community, financial, social, and emotional resources.
Our Private Homecare Program offers assistance with personal care, companionship services, transportation to medical appointments and social events on or off the Island, new baby care and much more. This program was designed to offer an “extra layer” of security for those seeking additional help that is not covered by Medicare or when Medicare coverage has ceased.
Osteoporosis Management and Prevention programs provide community access to screening, risk assessment, education, and supervised exercise.
Our Pain Management Program - Palliative Care - is active compassionate care of the chronically and terminally ill, primarily directed towards improving the quality of life when the goal is not a cure. The emphasis of palliative care is on the control of pain and symptoms and meeting the physical, emotional, spiritual, social, and cultural needs. It is multidisciplinary in approach encompassing the person, family, caregivers, and community in its scope.
