Home Visits

Home visits by our community health nurse are scheduled by appointment. The nurse is responsible for assessing the individual’s health, provide public health information aimed at improving the individual’s health, and directing them to appropriate providers if the nurse recognizes some issues that need further investigation and diagnosis.

“As a VNA homecare nurse you get to spend time with patients where they feel most comfortable-in their homes. I love having the time for patient teaching and to see the follow through of the benefits of my teaching. In home care there is a “continuity of care” and I enjoy this.
