The Vineyard Nursing Association is Martha's Vineyard's only Medicare Certified Home Health agency, providing a full complement of Home Health Care Staff who deliver customized health care for short term, long term and chronic illness through the following services:

Skilled Nursing All of our nurses are either Registered or Licensed Nurses. They provide not only the highest quality, compassionate health care at home but also support and instruct you in caring for a loved one at home. Their goal is to help patients live comfortably as they recover from or live with an injury or illness. Many of our nurses have been trained and accredited as Certified Pain Management Specialists.

Physical Therapy Our therapists provide general therapeutic exercises for ortho/neuromuscular development and improving balance, gait, and transfers. The therapists teach family members to assist in exercises and offer guidance in obtaining and using appropriate adaptive equipment.

Occupational Therapy
Occupational therapists provide general therapeutic exercises for patients with an injury or illness affecting perceptual motor skills or the ability to perform activities of daily living such as dressing, eating, or cooking. As part of the initial assessment we evaluate your individual situation, make recommendations for adaptive equipment  and recommendations for the  therapeutic exercises and appropriate training.

Home Health Aide Care
Most people need help with day-to-day tasks if you are recovering from or living with an illness. Our Home Health Aides make everyday living easier for persons of all ages. Our health aides provide assistance with basic needs for living at home such as bathing, dressing, and other personal care needs as well as meals and homemaking services. Our nurses, as part of your care plan, will determine the appropriate and allowable support that you will need during this challenging time. They will also monitor the quality of the aide care provided to ensure that any care provided by VNA aides meets our high standards.

Medical Social Work A licensed Social Worker will asses the needs of patients and families and present them with emotional support strategy and referrals to appropriate community services. We typically refer to community services related to coping skills, mental health issues, safety, financial concerns, housing options, counseling, long-term planning, and more.

Who's Eligible?
Do you wonder if you are a patient that is considered "homebound" as defined by Medicare? Consider this...

1. Being homebound does NOT mean that you never leave home.  The definition of homebound is that it ordinarily takes a "considerable and taxing effort to leave the home without assistance."

2. Homebound can be ordered by a physician for any period of time for medical or safety reasons.

3. Homebound does not need to be forever; it can be a reasonably short term.

4. Homebound does not prohibit you from leaving home to go to church, medical appointments, for a ride in the car, out for a meal or for personal care services.

5. The need to have another person with you in order to leave home is considered a taxing effort in most cases.

6. The need to use a device to walk - such as a walker, cane, crutches or a wheelchair - is considered a taxing effort.

7. If you can walk unattended, but become short of breath and need frequent rest stops while walking, is considered a taxing effort.

8. If you have some level of confusion and should not leave home alone, that is also considered homebound.

9. Medicare has changed the criteria for homebound.  Call us today for an assessment.

10. Homebound does not mean bedbound.
