Nurses – Salaried case manager RN and per diem RN or LPN positions

Job Description/Responsibilities:
As a home health nurse with the VNA, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery. You will maintain ongoing communication with the Clinical Supervisor, relaying any patient needs or problems. We have a salaried case manager RN position (with benefits) requiring every other weekend and several per diem positions available. Additional responsibilities include:

  • Making initial nursing evaluation visit.
  • Continually evaluating the patient’s needs.
  • Initiating, coordinating and revising the treatment plan.
  • Rendering nursing care and performing treatments, such as IV administration, as ordered by a physician.
  • Participating in the Quality Assurance/Quality Improvement Process.
  • Preparing accurate, orderly and timely reports and documentation.
  • Adhering to all practice standards as they apply to patient care.

Job Requirements:
You must be a caring, patient and flexible teacher with excellent communication and time management abilities. You must possess independent problem solving abilities, excellent clinical assessment skills and be computer literate. You must also be in good physical shape and have the ability to lift/transfer patients to and from wheelchairs, if necessary. Additional requirements include:

Current nursing license in MA.
Acute nursing care experience preferred, home health care experience a plus.
Methodical and tidy work habits.
Excellent oral and written communication and interpersonal skills; computer competency.
Ability to assess patient needs to develop and implement comprehensive plan of care​.
Ability to function as a member of an integrated healthcare work team.
Reliable vehicle and valid driver’s license with clean driving record required, as well as proof of auto insurance.

Physical Therapist – Per diem position

Essential Duties and Responsibilities:
Performs initial and on-going assessment to determine level functioning
Develops and revise the plan of care in consultation with the physician and other care-team members.
Provides therapeutic treatments and evaluation of equipment needs as applicable.
Prepares timely and appropriate clinical assessment and progress notes in accordance with accepted documentation standards.
Advises and consults with the family and other agency personnel, as applicable.
Evaluates patient in the home setting by applying muscle, nerve, joint and functional ability test.
Identifies patient’s physical therapy needs for home care and develop long and short goals to meet those needs.
Applies physical therapy procedures to achieve goals and re-asses the patient periodically, as needed.
Records and reports the patient’s reactions and conditions to physicians and agency personnel per the interdisciplinary communication policy.
Answers questions regarding physical therapy related to present and future care.
Works in interdisciplinary model to integrate physical therapy visits into the overall plan of care.
Performs other duties as required.

Job Requirements:
Graduate of an accredited college or university with minimally a bachelors degree in Physical therapy
Actively licensed therapist in MA as a Physical Therapist
One year experience as PT in home health, acute care or rehabilitation setting
Strong interpersonal, oral and written communication skills
Knowledge of Medicaid/Medicare regulations
Ability and willingness to travel locally
Valid driver license, registration and automobile insurance
Successful completion of a physical therapy program at a school of physical therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association.
Medical Social Worker – Per diem position

Job Description/Responsibilities:
The Medical Social Worker provides, as prescribed by a physician, psychosocial assessments, diagnosis and treatment to patients in their place of residence to assist the patient/family member(s)/caregiver(s) to develop effective coping skills in response to the patient’s diagnosis, treatment and/or life situation. The MSW assists/supports patient and caregivers in identifying and accessing resources in the community. This position serves as a team member with nurses and rehabilitation clinicians working toward comprehensive patient recovery.

Job Requirements:
You must be a caring, patient and flexible teacher with excellent communication and time management abilities. You must possess independent problem solving abilities, excellent clinical assessment skills and be computer literate. Additional requirements of our Medical Social Worker include
Current MA license.
Master’s degree from a school of social work accredited by the Council on Social Work Education, LICSW preferred.
Two (2) years post-masters experience in a clinical setting.
One (1) year of social work experience in a healthcare setting.
Prior home health experience preferred.
Able to communicate effectively in English, both verbally and in writing; additional languages preferred.
Knowledge of regulations/standards applicable to home healthcare.
Knowledge of general medical social work modalities/interventions and scope of medical social work practice within the state of Massachusetts.
Reliable vehicle and valid driver’s license with clean driving record required, as well as proof of auto insurance.
